APPLICATION AND ORDER FOR APPROVAL AS SPOKESPERSON

  

The undersigned hereby applies to the Court for approval to act as spokesperson.

4.         Applicant is/is not an enrolled member of Coquille Indian Tribe.

5.         Applicant certifies as follows:

A.  I have read, I understand and I agree to abide by the Constitution of the Coquille Indian Tribe and the following chapters of Coquille Indian Tribal Code:  610 (Tribal Court Ordinance), 620 (Coquille Rules of Civil Procedure), 630 (Coquille Evidence Code).

B.  In addition, if the case is a case involving a juvenile, I certify that I have read, I understand and I agree to abide by the provisions of Coquille Indian Tribal Code chapters 640, 641, 642, 643 and 644.

            I certify under penalty of perjury that the above statements are true.
